New Patient Registration

If you are not already registered as a patient at the Terrington Surgery, please bring the following items with you to the surgery in order to process your registration

  • your repeat medication list
  • details of your last doctor (name and address)

At registration you will be automatically allocated to one of the doctors, however if you have a preference for a specific doctor please state so on the form.

We will also offer you the opportunity to make an appointment to see our practice nurse for a new patient check-up or to discuss your medication. However, if you have a complicated medical history it might be more appropriate for you to have this checked by the doctor.

You may print off the following bring them into the surgery already filled out to complete your registration with Terrington Surgery.

It would be useful if you could also bring a list of your current medication as provided by your previous GP.
